The woman was later diagnosed to be found suffering from a major health complication called Ovarian Hyperstimulation Syndrome (OHSS).
What is Ovarian Hyperstimulation Syndrome?
Ovarian Hyperstimulation Syndrome(OHSS) is becoming a popular infertility complication in women. To cure the abnormality in infertile women, experts recommend giving stimulating hormones which produce several eggs. In case if this procedure is not properly monitored it causes developing Ovarian Hyperstimulation Syndrome(OHSS).
Super-ovulated ovaries or several eggs that are produced as a result of the drug are loaded with the estrogen hormone. The ovaries are flushed with sufficient amounts of oestrogen-rich fluid right through the abdominal cavity. The oestrogen rich fluid has chemical elements which form the lining of the abdominal cavity (called the peritoneum) which allows it to absorb the fluids poured meaning permeable (leaky). Upon absorbing the fluids, the ovaries increase in size causing dizziness, causing low BP.
Can this be blamed purely on IVF or infertility treatments on the whole?
In some abnormal cases of multiple pregnancies, where women have no access to proper medical treatment and no proper doctor supervision. It can be assumed in 2 ways: It could either be that the woman was given IVF treatment and doctor might have transferred all the 10 embryo which does not happen in good hospitals, doctors select the most viable embryos which are then transferred for implantation. Also, it could happen that the woman was given the IUI (intrauterine insemination) where several eggs are produced and the doctor instead of stopping the treatment must have continued ahead.
How can a woman stay assured that she has no chances of complications when she undergoes Hyperstimulation?
There is nothing much to do to rule out this complication. OHSS is an iatrogenic complication caused due to the hormones or medications are given. Having said that, under the guidance and monitoring of experts the risk of developing is fairly low.
What can the treating physician do to avoid this complication?
The best way is to observe the progress of the patient. If the doctor observes that the patient has grown too many follicles on ultrasound scanning, he should cancel the cycle, rather than continue with the treatment.
What factors mitigate this complication?
There are several things that can lead to this complication, but some biological and man-made factors can also lead to it. Some of them include:
1. PCOS: Women suffering with PCOS have a greater or increased probability of developing OHSS, as their bodies respond too much to the fertility medicines.
2. Poor medical conditions and monitoring: When giving such a treatment the doctor has to closely monitor the patient to avoid serious complications.
3. Lack of care when using extremely high doses of powerful HMG injections (Human Menopausal Gonadotropin – to stimulate the production of eggs by doctors who are less experienced and have no knowledge in case of any complications.
4. Poor awareness: Doctors who lack experience in such serious treatments must be avoided at all times.
